Additional Availability Check Form Feilds

Hello All,

Is there any way I can include more form feilds in the return availability form. Currently the form allows you to write
Surname: *
Name: *
Email: *
Nation:
Telephone:
Comment:
or Check again.

I would like to include a payment method in this form. If anyone can help, I would be obliged.

Try this way:

In the main menu: Configure & customize > Internet Templates

1. Create a new Internet template.
2. Under Form fields to be asked you'll find Custom form fields.
