Add custom form field, but doesn't show

Hi, I have another question, just a simple question. I add a new Custom Form Field from the template menu and click the "Create Availability Template" button already. But I don't see any field that I just created, so am I missing any step?

Are you opening the template you've just created or a copy of it you made somewhere else?
